Collaborating across teams

Working collaboratively across teams is critical for quality patient care, but it can be challenging. Working in silos can prevent clinicians from compiling patient data, which can put patient care at risk. Care planning provides clinicians with data-driven plans of care, including multidisciplinary clinical pathways and care protocols. Adding in expected outcomes and interventions allows for flexibility in customizing plans for patient-specific needs, enabling nurses to facilitate care that considers multiple aspects of a patient’s well-being.

Connecting care across the continuum

Patient data collected at one venue isn’t always accessible at other venues. This may lead to repeat tests, missing patient data and incomplete medical history, which can impact patient care. By making patient data shareable among physicians, specialists and patients, we’re helping to eliminate barriers to effective care. One of the most practical applications of health IT involves increasing access to information that’s already collected.

Safety

Medication administration

Our technology is designed to help keep patients safe from medication errors. Using barcode scanning, clinicians can read the patient’s name band and the medication before administration, which contributes to preventing medication errors. The technology is also integrated into infusion pumps that communicate with clinical documentation to capture the right volume and rate without manual intervention. With this technology, organizations may catch mistakes before they occur.

Infection isolation

Nurses are on the front lines of patient-centered care, where both the patient and nurse are susceptible to infectious disease. However, it can be difficult to identify the symptoms of the deadliest pathogens. Cerner developed the St. John Sepsis Agent to screen high-risk patients early in the infectious process while increasing accuracy in estimating mortality risk, enabling clinicians to deliver medical treatment promptly.

Patient monitoring

Falls are the most frequently reported adverse events among adult patients. It can be difficult and costly for nurses to monitor patients with a low risk of deterioration. For patients with fall risks or other intermittent medical conditions, vigilance in critical moments can help avoid dangerous events. With Cerner Patient Observer™, nurses can rest assured that in the event of an emergency in a patient room, the monitoring technician can quickly request help.

Communication

Going where nurses go

Health care doesn’t stop and nurses need access to solutions wherever care is needed. To keep patient-centered care moving forward, nurses can use CareAware Connect™ to send a secure message to care team members asking them to review a patient’s status remotely if they are not on site.

Communicating across teams

Our commitment to providers encourages smoother communication across multidisciplinary care teams. Our core mobile solution can display patients' care team members along with an option to contact them, enabling them to reach the right clinician. Our labor and delivery FetaLink+® solution saves time by allowing the nurse and physician to review the same data.

Reducing alarm fatigue

Nurses sometimes experience more than 350 alarms per patient, per day. As a nurse juggles multiple patients and tasks throughout a shift, it can be difficult to keep track of the noises and demands coming from every direction. Preventing nuisance alarms and recognizing meaningful alarms is key to reducing alarm fatigue. With our intelligent mobile solutions and client collaboration, only actionable, critical alerts get pushed to a nurse’s attention.

Calculating a patient’s acuity

Our solutions have helped health care organizations calculate patient acuity, specifically the care hours needed based on information captured in the EHR. This has helped balance the high acuity patients between nurses, thus distributing fair and manageable workloads. In the critical care environment, effective measurement and reporting of outcomes is essential. Cerner APACHE® Outcomes allows a nurse to objectively evaluate the quality of care delivered in an ICU setting. With a focus on process improvement and resource optimization, APACHE provides access to the one of the largest outcomes databases in the world, containing more than half a million contemporary admissions, with the greatest depth of benchmarking capabilities.

Planning for discharge

Seamlessly moving patients throughout the hospital as their needs change is a team effort. Nurses play a critical role in ensuring the appropriate level of care is delivered to each patient. Our centralized operations approach includes bed management from CareAware Capacity Management® and digital unit dashboards in the logistics center, aligning nursing across the hospital with EVS, transport, care management and other team members.
